Up to 13 positions can catch fire, triggering Fire Frames. Symbols in those positions split in two. All 5 reels may also catch fire, splitting symbols on the reels into 3 symbols.
Dead Patients
This feature triggers with at least two Dead Patient premium symbols, revealing a symbol multiplier between x5 and x9,999 for one of the dead patient symbols. The multiplier equals the number of instances of that symbol.
Enhancer Cells
Enhancer Cells below reels 2, 3, and 4 activate when Fire Frames are present. You get 1, 2, or all 3 Enhancer Cells activated from 4, 6, or 8 active Fire Frames, with Fire Reels counting as 2 or 3 Fire Frames. The Enhancer Cells may deliver the following:
- Wild Symbol - substitutes for any pay symbol.
- xSplit Symbol - splits a symbol on affected reels and splits itself into 2 wilds.
- xNudge Wild - nudges to fill the entire reel while increasing the win multiplier by +1 per nudge. If split by an xSplit, the multiplier increases by +1.
- xWays Symbol - expands the reel height by revealing 3 matching symbols. All xWays symbols reveal the same symbol if more than one is activated.
xHole
The xHole feature triggers when the xHole symbol appears inside a Fire Frame or on a Fire Reel on the second position from the top on the middle reel 3. The xHole sucks in pay symbols and wilds, re-adding them with boosted sizes of x2, x4, or x8. Affected xNudge Wilds get their multiplier boosted but remain in place.
xMental
The xMental feature triggers when the xMental symbol appears in a Fire Frame or on a Fire Reel, triggering an extra spin where all positions catch fire. The Enhancer Cells are all activated and you also have an increased chance of triggering the xHole feature. However, if you trigger the bonus round on the same spin as the xMental symbol, the xMental feature does not activate.
Mental Transform
The Mental Transform feature triggers when you land the Disembodied scatter symbol on reels 2 and 4, turning the scatters into xSplit, xWays, Dead Patient, Wilds, or regular Patient symbols.
Scatters
Different scatter combinations trigger different bonus rounds. You win +1 extra free spin per scatter that lands in a Fire Frame during either free spin feature, and +2 extra spins per scatter that lands on a Fire Reel.
Bloodletting Free Spins

Our Verdict
As a reviewer who tests new slot games, few have made an impression like the original Mental. While the shock effect isn’t as hard this time, Mental 2 delivers everything you could hope for from a sequel.
The Hannibal Lecter-inspired character is not the main focus, although he’s still present as the xGod symbol. The graphic content warning should not be taken lightly. The big win story depicts gruesome scenes of a young girl murdering her sleeping parents with a knife. You still can’t help but feel for her when you see her sad face on the reels, but the high-energy soundtrack helps.
The core features have been carried over and enhanced. You’ll benefit from Fire Reels, and one extra Enhancer Cell. xHole and xMental features are welcome additions. Eligible players have more to choose from in the Bonus Buy and Booster menu, including the pricy God Mode spin.
Mental 2 is even more brutal than its predecessor, clocking in at 12 out of 10 on the volatility scale. The max win has been bumped up to 99,999x, while boasting a higher hit rate of 1 in 16m spins. The bonus round hit rate is slightly lower, but still a decent 1 in 269 spins. Mental 2 is the gut-wrenching sequel I dreamed of.
